2 other things the above groupings have in common are 1. Population density and 2. Air pollution. The 3 higher medicare cost areas have much higher population densities (some of the highest in the US) and 2 of the 3 are on the ALA&#039;s listing for &quot;worst air quality&quot; in the US. The lower cost areas are ranked by ALA as in the top 25 cleanest air quality and have much lower population densities. Taking vulnerable populations ( poor and elderly) and placing them in such conditions almost begs for higher costs and worse outcomes. In fact if out comes were the same in each area the added expenses would surely be justified. The same can be shown for ER utilization rates (which are independent of PCP/Population ratios).]]></description>
